Principal Investigator: Robert Gerwin, MD
Role: Robert Gerwin conceived of the study, developed the protocol, including the outcome measures, arranged for the funding, and coordinated the research team. Dr. Gerwin also was primary of two physicians who performed the clinical evaluations. The statistical analysis of the first phase of the study was interpreted by Dr. Gerwin. Dr. Gerwin will be the principal author of the paper that will be written based on this study.
Per cent effort: This study took between and 5-10% of Dr. Gerwin’s time during the active phase of study development and the clinical phase of the study. It is estimated that about 25% of the actual time required to conduct the study was performed by Dr. Gerwin.

Pain Fellows from the Blaustein Pain Clinic, Johns Hopkins, 1995-1996, came to my clinic to learn about myofascial pain diagnosis and management, including Dr. Lee Ann Rhodes, now director of the Pain Anesthesia Clinic at Washington Hospital Center, Washington, D.C.
Wannee Ketmalasiri, M.D., of Bangkok, Thailand, spent 2 months studying with me in 1999.
Cesar Fernandez de las Peñas, PT, Madrid, Spain, studied headache and myofascial pain with me for two months in 2005, as a fellowship grant recipient of the International Myopain Society. He is now receiving his PhD under Lars Arendt Nielsen in Copenhagen, Denmark
Dr. Alex Karlov, a Russian neurologist from Moscow, studied the myofascial aspects of head and neck and low back pain with me for 6 weeks in 1994.
Orlando Mayoral del Moral of Toledo, Spain, worked with me at my clinic for several weeks and again in Spain, in the 1990s. He has since established a graduate program for physical therapists at the University of Catalonia and La Mancha, Toledo, Spain.
Steven Stavro of South Africa studied musculoskeletal pain syndrome treatment techniques, and the administration of botulinum toxin in painful musculoskeletal disorders with me for one week in 2006.
Tova Scott of Israel also studied diagnostic and treatment techniques with me for one week in my clinic. An Australian Doctor whose name I no longer remember did the same for one week in the 1990s.
Dr. Alberto Bedendo, rheumatologist of Padua, Italy, has studied with me in my clinic, off and on for several years, learning aspects of differential diagnosis and treatment techniques of soft tissue pain syndromes.
Dr. William Teachy, otolaryngologist of Virginia Beach, VA, worked with me off and on for several years, on the problem of head and neck pain syndromes. He has presented his work nationally, and we are collaborating on a paper on this topic together.
Stella Emsellem, 3rd year medical student at Des Moines, Iowa School of Osteopathic Medicine, June 22-25, 2009. Ms. Emsellem spent a one week externship with me in my clinic, to learn management of musculoskeletal pain problems.
I have tutored many other physicians and therapists in my clinic for periods of days to a week or two, whose names I did not record and can no longer remember. For example, Dr John Jarrell, gynecologist, attended my workshop in Calgary, Canada and then worked with me in Bethesda, Maryland. He has since published papers on the treatment of abdominal wall trigger points in the successful management of endometriosis-related abdominal pain.
